Summary

We are seeking a warm, compassionate and proactive individual to join our team as Support Coordinator, supporting older individuals at our Extra Care service in Dukinfield, Tameside.

The scheme consists a number of one bedroom and two bedroomed self-contained flats, with a proportion of individuals receiving a care service. Needs may include dementia, mental health needs, physical disabilities, learning disabilities, and complex health needs. You will also assist the Extra Care Manager and Registered Manager in ensuring an empowering service which meets the needs of our clients and works with the service user’s families and friends to promote a positive atmosphere.

Creative Support is committed to your continuous professional development and as a successful candidate; you will receive a full local and corporate induction, and attain a level 3 certificate in ‘Preparing to Work in Adult Social Care’. On completion of your probationary period, you will automatically be enrolled onto the Level 3 Diploma in Health and Social Care. You will also have the opportunity to discuss your training and development needs during regular supervision and appraisal meetings.
