Summary

As a Band 6 practitioner, you will work with a high degree of professional autonomy and accountability, taking a lead role in the assessment, formulation and delivery of care for people experiencing an acute mental health crisis. Main duties include: Act as an autonomous practitioner, undertaking timely, comprehensive mental health assessments in the community and formulating presenting needs, mental state, capacity, physical health, safeguarding concerns and dynamic risk. Being an active participant in a shared caseload during episodes of intensive home treatment, developing, delivering and reviewing collaborative, recovery-focused care and safety plans with service users and, where appropriate, carers. Use advanced clinical judgement to deliver evidence-based interventions, monitor response to treatment and adapt plans promptly when presentation or risk changes. Undertake crisis triage and contribute to gatekeeping assessments for psychiatric admission, ensuring that viable alternatives are considered and that decisions are clearly documented, defensible and based on the least restrictive safe option. Make and communicate sound clinical decisions in complex or rapidly changing situations, escalating significant concerns appropriately and supporting coordinated responses with emergency and statutory services. Administer, monitor and promote the safe use of medicines, and undertake relevant physical health assessment and monitoring within professional scope, competence and Trust policy. Coordinate care across the multidisciplinary team, inpatient wards, community services, primary care, social care, emergency services and voluntary-sector partners to support admission avoidance, continuity of care and timely discharge. Maintain accurate and contemporaneous clinical records, provide clear handovers and reports, and contribute actively to multidisciplinary reviews, safeguarding processes and Mental Health Act pathways. Provide day-to-day clinical leadership, advice and role modelling to junior colleagues, students and support staff; contribute to induction, supervision, reflective practice and the development of a positive learning culture. Participate in implementing clinical audit action plans, being an agent for change in quality improvement and service development projects, applying learning to improve safety, experience and outcomes. Coordination of the shift as allocated duty worker, prioritising workload and supporting safe allocation of team resources. Participate in the 24-hour, seven-day rota, including evenings, nights, weekends and bank holidays, in line with service requirements.
