Package Description: Are you an experienced Healthcare Assistant looking for an opportunity to develop your clinical skills while making a real difference to patients within the community? Were looking for a compassionate and motivated Band 3 Community Healthcare Assistant to join our Community Nursing service. Youll work directly with patients in their own homes and residential care settings, delivering nursing care delegated by our registered nursing team. Its a varied and rewarding role where youll have the opportunity to build relationships with patients and carers while developing a broad range of community healthcare skills. Depending on your training and competencies, your work may include phlebotomy, wound care and dressings, blood glucose monitoring, continence support and other delegated clinical interventions. Youll be trusted to organise and manage your delegated visits, working independently in the community while remaining supported by experienced District Nurses and Senior Nurses. Youll also play an important role in observing patients, recognising changes in their condition and ensuring concerns are communicated promptly to the appropriate clinician. Package Description As a Band 3 Community Healthcare Assistant, youll be part of a valued Community Nursing team, delivering high-quality care to patients within their own homes and residential care settings. You will feel valued within HCRG Care Group, receiving access to exclusive rewards and benefits including: Membership of My Reward Hub, giving you access to discounts on everyday purchases such as grocery shopping, as well as cashback and voucher offers Access to your wages as you earn them to help cover lifes emergencies and avoid overdraft fees or high interest rates Online and face-to-face support with your mental and physical wellbeing, from healthy recipes and activity challenges through to post-trauma support, legal, debt and life management help, career coaching and counselling Access to eLearning, bespoke career pathways and opportunities for continuing professional development through our Outstanding learning and development team, The Learning Enterprise An open, just culture where youre encouraged to share ideas and contribute to improving the way we deliver care The opportunity to develop your community nursing skills through training, supervision and ongoing professional development Main Responsibilities As a Community Healthcare Assistant, you will: Deliver high-quality nursing care to patients within their own homes and residential care settings Carry out delegated clinical activities in line with your training, competencies and agreed care plans Support patients with wound care and dressings, blood glucose monitoring, continence care and other appropriate clinical interventions Undertake phlebotomy following appropriate training and competency assessment Observe patients and identify changes in their condition, reporting concerns promptly to the appropriate nurse Organise and prioritise your delegated visits according to patient needs Work independently in the community while recognising when advice or additional clinical support is required Maintain accurate, clear and timely patient records, including electronic patient records Provide compassionate support and communicate effectively with patients, carers and families Work collaboratively with District Nurses, Senior Nurses and other members of the multidisciplinary team Contribute to patient safety by recognising and reporting incidents, risks or concerns appropriately Participate in team meetings, clinical supervision, training, appraisals and ongoing professional development Support the induction and orientation of new colleagues and students where appropriate Assist with administrative and clerical activities required to support the effective running of the service Help ensure equipment and resources are maintained and used effectively Maintain patient dignity, privacy and confidentiality at all times The Ideal Candidate Youll be a compassionate, reliable and proactive Healthcare Assistant who enjoys working directly with patients and appreciates the variety and independence that comes with community-based care. Youll need to be confident working on your own during delegated visits while also understanding that youre part of a wider clinical team and knowing when something needs to be escalated. You will have: NVQ Level 2 in Care or an equivalent relevant qualification Previous experience working within health or social care Strong communication and interpersonal skills The ability to communicate with patients and carers in a compassionate, respectful and reassuring way The ability to work effectively both independently and as part of a team Good organisation and prioritisation skills Confidence using IT systems and electronic patient records The ability to recognise changes in a patients needs or condition and communicate these appropriately A willingness to undertake training and develop your clinical competencies The flexibility to work across a seven-day service An interest in developing your career within community nursing
