Job description

Social Worker – Wrexham Children Services Leaving Care Team.

Social Work qualification must be held.

The Leaving Care Team work with care experienced children and young people aged between 15-25. The focus is to support with transition planning through to independence. You will be committed to corporate parenting, relationships-based practice as the driver of positive outcomes and transitions. Trauma informed approaches will be at the heart of your practice. We will also expect you to model best practice and mentor workers within the team. You will be joining an established, high performing team with excellent support. Complete LAC reviews and statutory visits and have an understanding of legislation in Wales. You will be expected to work from the office and able to work from home. This is a temporary post for 6 months.
