Summary

To coordinate the day-to-day delivery of care services, ensuring care packages are appropriately staffed, safely delivered and effectively monitored. The postholder will manage daily rotas and care runs, allocate care workers, respond to operational issues and act as a link between care workers, clients, families, professionals and the management team.

Key Responsibilities

Manage daily and weekly rotas and care runs, allocating care workers according to client needs, skills, availability and continuity of care. Identify and resolve staffing gaps and arrange appropriate cover for sickness, annual leave and unexpected absence.

Take and respond to operational calls from care workers, clients, families and professionals, providing appropriate guidance, recording significant information and escalating urgent or complex matters to the appropriate manager.

Coordinate care packages and monitor the delivery of commissioned care hours, identifying missed visits, staffing issues, changes in client needs or concerns about the quality of care and ensuring appropriate action is taken.

Liaise with clients, families, social workers, health professionals and other agencies, ensuring relevant information is communicated promptly and accurately to the care team and management.

Contribute to the development, review and updating of care plans and risk assessments, ensuring changes in needs or circumstances are recorded and communicated to staff. Escalate significant changes to the Home Care Manager or Registered Manager.

Monitor care delivery, care records, medication records and other relevant documentation, contributing to audits, spot checks, quality monitoring and service improvement activities.

Identify and escalate safeguarding concerns, accidents, incidents, poor practice and other risks in accordance with organisational procedures, supporting appropriate follow-up and ensuring actions are recorded and monitored.

Contribute to the organisation's governance and quality assurance arrangements by providing management with information on operational risks, staffing, incidents, safeguarding and quality concerns, participating in meetings and supporting the implementation and monitoring of improvement actions.

Provide operational guidance and support to care workers, promote person-centred care and ensure staff follow care plans, risk assessments, policies and procedures.

Maintain confidentiality, professional boundaries and accurate records and work in accordance with safeguarding, GDPR, health and safety, equality and person-centred care requirements
