We are recruiting 2 new Band 6 registered clinicians to join our expanding Care home discharge liaison team to support the safe and successful discharges for complex mental health patients adult and older adult to 24 hr placements.
The discharge liaison service is being established in order to improve the discharge pathway for patients with severe enduring mental health or cognitive impairment requiring 24 hour placement, where discharge could be delayed due to difficulties in identifying a care setting with the capacity, capability and competency to manage their particular complex mental health needs. Clinicians within the team will be working with patients and wards once a 24 hour placement has been determined as discharge destination creating person centred care plans to maximise successful transition and placement into their new homes.
Main duties of the job
The role will focus on supporting discharge from acute mental health wards to the care homes. This will include supporting care home staff with advice and education regarding mental health, collaboratively creating personalised care plans to meet service user need and enable their care needs to be met by the placement. This will involve working closely with patients and their family/carers, ward staff, care home care staff, consultants and general practitioners, therapists and social services.
The post holder will deliver structured, person-centred care and evidence-based interventions continuing the care plan commenced in an acute setting. With the aim to promote mental health well-being on admission to care homes and a positive transition into long term care.
The service is a county wide service and the post holder may be asked to cover any localities within Oxfordshire.

Qualifications
Essential criteria
- Registered Mental health nurse or Occupational therapist with NMC or HCPC
- Evidence of continued professional development
Desirable criteria
- Post graduate education related to the client group and/or role
- teaching/facilitating qualification
Knowledge and Experience
Essential criteria
- Demonstrates significant post qualification experience in inpatient and or community.
- Experience of working within a multidisciplinary team and with other teams and agencies
- Strong clinical knowledge and expertise in mental health especially working with people with challenging behaviours
- Experience and strong understanding of risk management
Desirable criteria
- Experience of lone working and/or the ability to work autonomously
- Good knowledge of residential and nursing care home systems
- Evidence of up to date knowledge of the impact of pharmacological interventions on older people and those with long term health conditions
Skills
Essential criteria
- Demonstrates strong IT skills and able to maintain high standard of record keeping
- Reliable, team player that is able to work on own initiative
- Able to work within stressful situations
- Has a clean driving licence and able to travel countywide
Desirable criteria
- Experience of working with families and other agencies
