Summary
See attached document for full details. Primary Responsibilities: Clinical supervision and development of the Practice Nursing Team including such as HCAs / RNAs. Assisting in clinical audits and monitoring of standards of care. Assist in delivering the clinical QOF and Enhanced Services programmes. Provide core clinical practice nurse service covering the main clinical areas: health screening and risk assessment; cervical cytology; health promotion; vaccination and immunisations; infection control; child health/child protection, wound care / dressings, ECGs, and urinalysis. Provide care and programme management of chronic disease including, diabetes, asthma, COPD, hypertension, CHD, other vascular conditions. Provide care and programme management of lifestyle management including weight management and smoking cessation, mens health, family planning including pill checks and depos, patient medical checks. Chaperoning and assisting patients, where appropriate, who are being examined by another clinician Assisting GPs with minor surgery and coil/implant clinics if required. Requesting pathology tests, for example urine culture, swabs. Following agreed clinical protocols with referral to GPs as appropriate Monitoring and maintaining cleaning equipment used by the nurses and GPs; drugs and immunisation stock; clinical equipment stocks and volumes in all clinical rooms; general tidiness and cleanliness of treatment and clinical rooms. Participation in administrative systems in the Practice. Undertake all associated clinical data entry consultations, referrals, etc maintaining high standards of accuracy. Attend and participate in any Practice meetings when required. On occasion there may be a requirement to undertake home visits. Daily audit of fridge temperature checks and managing associated outcomes. Assist in and perform routine tasks related to patient care as directed by GPs. Organise Child immunisation appointments, review DNAs and recall. Support the Lead Practice Nurse in their duties. Any other delegated duties appropriate to the post.
