Package Description: Are you an experienced health or social care professional with a passion for reablement, leadership and helping people maintain their independence? Were looking for an Enabler Supervisor to join our Intermediate Care and Reablement Service, helping us deliver high-quality care and support to people within the community. This is a varied, hands-on supervisory role where youll lead and support a team of Enablers while ensuring service users receive care that is safe, personalized and focused on achieving their individual goals. At the heart of the role is reablement. Youll help ensure our teams focus on what people can do and want to achieve, encouraging service users to regain skills, confidence and independence wherever possible. Alongside supporting service users, youll provide day-to-day leadership to Enablers through supervision, coaching, observed practice and spot checks. Youll help colleagues develop their skills, maintain high standards and feel confident delivering care within the community. Youll also work closely with therapists, clinicians, Adult Social Care and other partners, helping coordinate care and responding effectively when needs or circumstances change. Package Description As an Enabler Supervisor within our Intermediate Care and Reablement Service, youll play a key role in supporting people to regain their independence while leading and developing the colleagues who provide their day-to-day care. Youll be part of a supportive multidisciplinary service, working closely with Enablers, therapists, clinicians, social care colleagues and other partners to make sure our service users receive safe, personalized and effective care. You will feel valued within HCRG Care Group, with access to a range of rewards and benefits including: Group Pension Membership of My Reward Hub, giving you access to discounts on everyday purchases such as grocery shopping, as well as cashback and voucher offers Access to your wages as you earn them to help cover lifes emergencies and avoid overdraft fees or high interest rates Online and face-to-face support with your mental and physical wellbeing, including healthy recipes, activity challenges, post-trauma support, legal and debt support, career coaching and counselling Access to eLearning, bespoke career pathways and opportunities for continuing professional development through our learning and development team, The Learning Enterprise An open, just culture where youre encouraged to share ideas, improve services and help us deliver high-quality care Main Responsibilities As an Enabler Supervisor, you will: Supervise, support and develop a team of Enablers to deliver safe, effective and person-centered care Provide regular supervision, observed practice and spot checks to maintain high standards Support the induction and development of new colleagues and help existing team members develop their skills and competencies Ensure individual care and reablement plans are followed appropriately and remain focused on each service users goals Promote independence by encouraging and supporting service users to regain skills and confidence in everyday activities Ensure care respects each persons privacy, dignity, choices, cultural needs and communication preferences Build positive relationships with service users, their families and carers Undertake and contribute to risk assessments and ensure appropriate measures are in place to support safe care Recognise changes or deterioration in a service users condition and ensure concerns are escalated appropriately Work collaboratively with therapists, clinicians, social care colleagues and the wider multidisciplinary team Monitor standards of practice and help ensure learning from incidents, feedback and audits is embedded within the team Support investigations, safeguarding enquiries and quality improvement activity where required Work alongside administrative and operational colleagues to help coordinate staff and service-user requirements Respond effectively to staffing pressures and help ensure continuity of care when circumstances change Support the services on-call arrangements where required Maintain accurate and timely records and promote high standards of documentation across the team Promote a positive culture of learning, accountability, teamwork and continuous improvement The Ideal Candidate Youll be a compassionate and confident individual who can combine excellent standards of care with the ability to lead, motivate and support others. Youll understand the importance of reablement and be passionate about helping people achieve as much independence as possible. Just as importantly, youll know how to support colleagues to deliver consistently high standards while providing constructive feedback and addressing issues when needed. You will have: NVQ Level 3 in Health & Social Care or an equivalent qualification Experience working within community care, rehabilitation, intermediate care or an integrated care service Experience supervising and supporting care or clinical teams Experience working as part of a multidisciplinary team A good understanding of Intermediate Care and rehabilitation pathways An understanding of Home First and Discharge to Assess approaches Strong leadership and team-development skills Excellent communication and relationship-building skills The ability to effectively organize and priorities competing demands Confidence making decisions, solving problems and escalating concerns appropriately Good IT, record-keeping and report-writing skills A compassionate, resilient and adaptable approach Evidence of continued professional development Leadership or management training, or a willingness to undertake this as part of your development Most importantly, youll be passionate about delivering care that promotes independence, dignity and positive outcomes, while supporting your team to provide the best possible experience for every service user.
