Job Title: Skin Cancer Support Worker

This is an exciting opportunity to join our team in the Skin Cancer Specialist Nursing team.

This is a great opportunity for someone looking to provide holistic nursing support and care to skin cancer patients across 3 different Hospital sites.

You will play an important role in supporting these patients along with the nursing and medical teams right from diagnosis to treatment across various specialities.

The successful candidate will be responsible for independently but under the guidance and supervision of a Clinical Nurse Specialists (CNS), to enable and support patients who have a diagnosis of a skin cancer. The post holder will also be part of the wider Skin multidisciplinary team (MDT).

The support worker will act as a point of contact for patients with skin cancers. They will be able to support patients with access to personalised care, including holistic needs assessment (HNA), care planning and health and wellbeing information.

The role will involve supporting the CNS teams with triage of their patient phone calls, coordinate care and investigations.

Another key aspect of this role will be to manage and support the delivery of personalised care which will be provided for patients as they complete treatment and transition into follow up care. The CSW with the CNS will scope the need and feasibility of other supportive self-management interventions - for example, 'Health and Wellbeing events’.

To help set -up and support the Patient Initiated Follow Up Clinics going forwards.
