Summary
The overall aim of the service is to work with individuals to increase their knowledge, skills and confidence to manage their long-term conditions and care needs, This role initially will pay particular emphasis to diabetes, however all long term conditions will be supported. This is a clinical role and will involve care delivery with a behavioral change focus. You will work closely with individuals to empower independence with their health and social care needs and use the Goal setting approach to help achieve this. The post holder:- To work as part of a multidisciplinary team supporting the care of patients within the community. To undertake the administration of medications that are prescribed for patients on a regular basis. To promote self-care and education to patients. Be able to create a professional culture which is open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values. Clinical duties The post holder will work within an agreed framework to plan, implement, and evaluate clinical and/or therapeutic activities and patient responses to activity. Support and assist other staff members to deliver care as delegated and directed. Demonstrate highly developed physical skills obtained through practice and training e.g., accuracy, dexterity and coordination to complete tests and assessments within agreed pathways of care and manipulate equipment. The post holder will be able to make judgement involving facts or situation which may require analysis e.g., alter programmes of activities or making clinical decisions related to specific pathway of care. Able to identify risk or changes in condition and is responsible for acting; advises supervising professional and accurately record all interventions. Provides and receives routine or sometimes complex information using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent for care, record discussions in an accurate and timely manner in the individuals patient record. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role. Engages and actively involves the individual and, if appropriate, their family/carer, in their assessment, programme of treatment and/or care by using person-centred techniques to promote a culture of self-care, involvement and empowerment. Prioritises their own workload and understands the boundaries of their practice; may plan tasks for others e.g., patients, the workload of more junior staff. Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g., stock control, basic equipment cleaning, specific clinical tasks to support the smooth running of a clinic. To comply with the organisations Infection Prevention and Control requirements, including bare below the elbows dress code for staff with a direct care delivery role. To work as part of a multidisciplinary team supporting the care of patients within the community Professional Duties Have demonstrated the proficiency to administer medications and undertake annual medicines management training. To be aware of and adhere to Trust Policies relating to the control and administration of drugs. To ensure that high standards of care are given and maintained and to act when standards are not being maintained. To work within the policies, procedures and guidelines of Calderdale & Huddersfield Foundation Trust. To participate in regular staff meetings. To be flexible to work within areas or departments to meet the demands of changing service needs. To provide mentorship and preceptorship to newly qualified staff and students, new starters and Trainee Nursing Associates encouraging an effective learning environment. To provide support and supervision to Health care support workers to promote their learning and development within role. To receive delegated duties from the supervising RN and senior colleagues, accepting those that fall within their individual competency level adhering to the NMC Delegation and Accountability tool (NMC Code 2018). To delegate appropriately to other team members, including Health Care assistants, family members and other lay carers adhering to the NMC Delegation and Accountability tool (NMC Code 2018). Recognize and work within the limits of their competence. To take responsibility for own continuing professional development and performance. Ensure that a professional service and image is maintained at all times, thereby acting as a role model to all staff. Ensures the effective and efficient use of resources within their own sphere of responsibility. Takes responsibility for their own and others health and safety in the working environment. Assist the audit department with compiling audit questionnaires by using knowledge of areas to be audited. Use audit tools provided by audit department to collect data. Participate in group reviews of services and areas as requested. To monitor that all policies relating to medicines are followed. Work with the team to produce meaningful reports on yearly /ad hoc basis. To participate in regular reviews of medication errors and assist and decide action plans within the reviews. Leadership duties To communicate with the nurse in charge and collect data in order to participate in the smooth running and organisation of the department. To support junior and senior colleagues in their roles and be resilient to manage change. To support the department nurse in charge in risk assessment and minimization. Ensuring incidents and near misses are reported. To manage verbal complaints, inform nurse in charge and refer on when unable to resolve. To participate in the investigation of incidents/complaints as required. To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team. To recognize, prevent where possible, and manage areas of conflict, reporting to nurse in charge if unable to resolve. Plan, organise and prioritize own workload effectively. Demonstrates resilience when responding to challenge, change and difficult situations. Demonstrates clinical leadership in everyday practice through identifying creative and innovative solutions, engaging in leadership development appropriate to level and encouraging development as part of the team. Learning and Development Undertakes any training required to develop or maintain their proficiency within the clinical area and demonstrates competence within sphere of role/ practice. To undergo further training as necessary, and attend training as identified at personal development reviews. Reflects on and evaluates own practice and identifies areas of development by setting appropriate objectives via appraisal and clinical supervision. Contributes to and undertakes clinical audit. Assists with research project appropriate to clinical area. Is committed to delivering and sharing learning opportunities with students, team members and other agencies in order to develop self and others. Working as part of a Team Actively works towards developing and maintaining effective clinical and corporate working relationships both within and outside the Trust including other agencies, individuals, and higher education Institutes. Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships. Actively contributes to the multi-disciplinary team supporting the patient including communication with external care providers, voluntary support services and other agencies; acting as a credible source of information for other agencies/professionals involved in the care as required. Work as part of a multidisciplinary team including nurses, occupational therapists, healthcare assistants, medical staff, pharmacy staff. Managing Self Attend all mandatory training. Participate annually identifying, developing, and agreeing your own development plan with your line manager using the Trust appraisal process. Comply with all Trust policies, procedures, and protocols. Carry out duties with due regard to the Trusts Equal Opportunity Policy. Seek advice and support from your line manager whenever necessary. Seek and Participate in regular clinical supervision. Safeguarding All staff members have a duty to report any concerns they have about the safety or well-being of adult service users, members of their families, including children. Employees should be aware of their roles & responsibilities to both prevent and respond appropriately to abuse. They should undertake the safeguarding training required for their particular role.
