Summary

Personal Assistant needed in the Carmarthen area for Early teatime, and bedtime call.

8.45 hours per week, 14 visits over 7 days.

And 3 hours socialisation to go shopping and to the beach.

Outcomes to be met -

To feel clean and meet her hygiene needs.

To feel supported in her own home at a suitable time for her.

To have daily support due to her chronic health conditions.

How will these outcomes be met -

- Personal Assistant to introduce themselves on each visit preferred language is English although I do speak Welsh preferred Bilingual speaker.

- Personal Assistant to be aware that as a result of her stroke and arthritis, takes time to mobilise and complete tasks.

- Personal Assistant to support to have a bath using the salts and remedies ,has a shower sometimes instead of a bath.

- Personal Assistant to support to dry after bath/shower, and dress in clothes of her choice, in particular to support with her lower half garments. I may wish to shower also when pain/fatigue allows.

- Personal Assistant to provide reassurance and support as can feel anxious about falling.

- Personal Assistant to apply any prescribed creams.

- Personal Assistant to maintain dignity at all times and ensure wishes are heard and respected.

- Personal Assistant to be aware I can become fatigued at times and as a result of her pain levels and mobility, her function may fluctuate day to day.

- Personal Assistant to support with fresh meal preparation each week so as to support with a healthy diet, by weekly batch cooking.

- Personal Assistant to prepare meals/snacks and fluids on each visit.

- Personal Assistant to record and report any falls to the relevant health professionals..

- Personal Assistant to ensure has all items she needs around her prior to leaving.

- Personal Assistant to clear away any dishes used and clean areas of the home used prior to leaving.

- Personal Assistant to report any skin integrity concerns to the relevant health professionals.

- Personal Assistant to change any soiled bedding.

- Personal Assistant to empty the commode on each visit.
