Summary

Job Summary The Acute Infection Advanced Practitioner is a pivotal member of the Winter Infection Service, providing high-quality clinical assessment, diagnosis, treatment and evaluation of patients presenting with acute infectious illnesses. Working autonomously and collaboratively with GPs, nurses, pharmacists, HCAs and wider multidisciplinary team colleagues, the practitioner will deliver safe, evidence-based care in line with their professional regulatory standards. This role focuses on the assessment and management of acute respiratory infections, urinary tract infections, skin and soft tissue infections, and other same-day infectious presentations. The post holder will support admission avoidance, reduce unnecessary emergency department attendances, and contribute to the delivery of responsive winter services across the practice population. Purpose of the job The purpose of this role is to independently manage and provide advanced clinical care for patients presenting with acute infectious conditions, ensuring high-quality clinical decision-making and patient-centred care. The Advanced Practitioner will: Deliver comprehensive clinical assessments, treatment plans, prescribing and follow-up. Manage patients presenting with respiratory, urinary and skin infections. Work collaboratively with the multidisciplinary team to support safe, efficient patient pathways. Contribute to clinical governance, quality improvement, audit and service development. Promote self-care, health education and antimicrobial stewardship. Support winter pressure initiatives and admission avoidance pathways. Uphold professional standards and engage in continuous professional development. Key Responsibilities Clinical Responsibilities Undertake face-to-face consultations, telephone assessments and clinical triage. Assess, diagnose, treat and manage acute infectious presentations. Make autonomous clinical decisions regarding diagnosis and management. Request, interpret and act upon relevant investigations and diagnostic tests. Prescribe safely and effectively within scope of practice. Manage patients presenting with: Acute upper and lower respiratory tract infections. Suspected pneumonia. Urinary tract infections. Skin and soft tissue infections, including cellulitis. Other acute infectious illnesses suitable for primary care management. Provide appropriate safety-netting and follow-up arrangements. Refer patients to secondary care or urgent services when clinically indicated. Maintain accurate and contemporaneous records within EMIS. Support admission avoidance and same-day urgent care pathways. Professional Responsibilities Maintain active registration with the appropriate professional body (NMC, HCPC or equivalent). Participate in annual appraisal, CPD and revalidation requirements. Maintain and update knowledge in line with NICE guidance and local infection management pathways. Promote effective communication within the practice and wider healthcare system. Uphold confidentiality and comply with all practice policies and procedures. Act as a role model for clinical excellence and patient-centred care. Administrative Responsibilities Ensure all consultations and clinical activity are accurately documented. Complete coding and administrative tasks associated with patient care. Support operational delivery of the Winter Infection Service. Attend clinical meetings, audits and training sessions. Support the development of new services and clinical pathways. Research, Audit and Quality Contribute to quality improvement and audit projects. Support antimicrobial stewardship initiatives. Ensure clinical practice remains evidence based and aligned with current guidelines. Participate in service evaluation and winter service reviews. Support CQC compliance and governance activities. Communication Communicate effectively with patients, carers and members of the multidisciplinary team. Adapt communication style to meet individual patient needs. Participate in multidisciplinary discussions regarding patient management. Work collaboratively with community, secondary care and urgent care providers. Fire Warden and associated responsibilities. Chaperoning and assisting patients to consulting rooms Possess the ability to motivate, organise and prioritise your own workload. To undertake any other responsibilities, to maintain a high standard and efficient service.
