Summary
Job Title: Support Worker
Reports To: Registered Manager / Care Coordinator
Location: Middlesbrough, Stockton-on-Tees, Redcar & Cleveland
Hours: Full time, part-time, bank availability
Job Purpose
To provide safe, high-quality, person-centred care and support in individuals’ homes and the community, promoting dignity, independence, choice and wellbeing in line with care plans, risk assessments, company policies and CQC standards.
Key Responsibilities
- Provide person-centred care and support, including personal care, mobility, nutrition, hydration and domestic tasks where required.
- Support individuals to develop independence and access social, leisure and community activities.
- Administer or assist with medication when trained and assessed as competent.
- Provide emotional support, reassurance and companionship.
- Monitor and report changes in health, wellbeing, behaviour or risk.
- Maintain accurate care records, including care notes and MARs.
- Follow safeguarding, health and safety, infection control, confidentiality and other company policies.
- Work professionally with colleagues, families and other professionals.
- Attend required training, supervision and team meetings.
- Work flexibly, including evenings, weekends and bank holidays where required.
General Responsibilities

- Report safeguarding, health and safety and other concerns promptly.
- Comply with relevant legislation, including the Health and Safety at Work - - Act, Mental Capacity Act, GDPR and applicable CQC requirements.
- Undertake other reasonable duties as directed by the Registered Manager.
