Summary

An exciting opportunity has arisen for an experienced ACP to join our Home Visiting Wellbeing Team, working in and around Boston. You will be an enthusiastic and motivated ACP who wishes to influence patient care in this bespoke area of primary care. Your main duties will be to assist daily with home visits including visit triage, seeing on the day patients for acute and minor illness, for housebound and care home patients, working on behalf of Boston Primary Care Network (PCN) practices. You will be employed and supported by Boston PCN Ltd and will work with a diverse, multi-skilled, experienced professional team of clinical and non-clinical, friendly and committed staff. Boston PCN covers six practices across Boston, Kirton and Swineshead areas.
