Summary

As a Care Coordinator, you will: Assisting patients by directing patients to the most appropriate clinician or service. Dealing with appointment, home visit and telephone requests. Booking, amending, and cancelling patient appointments. Contacting patients to arrange annual health and wellbeing reviews, particularly for those with long-term conditions and patients who are housebound. Providing day-to-day administrative support, including processing letters, updating patient records, responding to patient enquiries and helping out on Reception when required. Encouraging patients who have missed their cancer screening appointments to attend by contacting them and helping arrange appointments. Arranging referrals to hospitals and community services, keeping track of their progress and following up where necessary. Supporting our vaccination programmes, including coordinating COVID-19 vaccinations for housebound patients and assisting with vaccine clinics when needed. Keeping patient records accurate, up to date and confidential using the practice clinical system. Speaking with eligible patients about NHS Weight Management Programmes and helping them access the right support. Working closely with GPs, nurses, the wider practice team and external healthcare providers to deliver joined-up, patient-centred care. Chasing referral appointments, reports and outcomes to help ensure patients receive the care they need without unnecessary delays. Following all NHS guidance, practice policies and information governance requirements to ensure safe and effective working.
