Summary

Hazelcare is looking for an organised, experienced and compassionate Supported Living Coordinator to help oversee our supported living services across Bristol, South Gloucestershire and North Somerset.

Our main office is based in Kingswood, Bristol.

We provide person-centred support to adults with a variety of needs, including mental health needs, learning disabilities and difficulties, substance misuse and complex support needs.

This is an excellent opportunity for an experienced Support Worker, Senior Support Worker, Team Leader or Coordinator who is ready to take greater responsibility and help lead high-quality services.

About the Role

As a Supported Living Coordinator, you will work closely with the Registered Manager and support staff to ensure our services run safely and effectively.

Your responsibilities will include:

Coordinating the day-to-day running of supported living services.

Supporting and supervising Support Workers.

Helping prepare and manage staff rotas.

Ensuring shifts are appropriately covered.

Providing guidance to staff regarding individuals’ support needs.

Completing and reviewing support plans and risk assessments.

Monitoring daily records and ensuring documentation is accurate.

Supporting people with mental health needs, learning disabilities, substance misuse histories and complex needs.

Supporting individuals to develop independence and achieve personal goals.

Monitoring medication processes and ensuring procedures are followed.

Responding appropriately to incidents and safeguarding concerns.

Conducting staff observations, supervisions and competency checks as required.

Supporting new staff during their induction.

Liaising with families, social workers, health professionals, commissioners and other agencies.

Attending reviews and professional meetings.

Supporting individuals to access their local community.

Completing quality checks and helping maintain high standards across services.

Reporting concerns and service developments to the Registered Manager.

Supporting the Registered Manager with service development and continuous improvement.

Providing on-call support where required.
