Summary
Are you passionate about making a real difference in people’s lives? Do you thrive on helping others overcome challenges and build brighter futures? The Nelson Trust is looking for a dynamic, compassionate Community Connector to join our Recovery & Aftercare team!
This role involves supporting clients who have completed treatment and need help to further develop their recovery capital. You will act as a bridge between structured treatment and aftercare, ensuring clients receive compassionate and collaborative services.
Salary: £27,000 - £28,500 per annum
Hours: Full Time (35 hours per week Monday - Friday)
Location: Countywide across Gloucestershire
What You’ll Do:
Conduct individual assessments and develop personalised recovery plans.
Provide crisis intervention and assist clients in accessing necessary services such as mental health support, housing assistance, and employment resources.
Facilitate participation in mutual aid groups and provide information on addiction and relapse prevention.
Monitor progress, advocate for clients, involve family members, and support volunteer activities.
Report all safeguarding concerns and support the delivery of recovery coaching and peer mentoring programmes.
Coordinate recreational activities and community events.
