Summary

We are looking for a Care Coordinator to join and grow our new domiciliary care company in Carlisle. The right candidate must have at least a year of experience in Domiciliary care, looking after children, adults, and elderly people in their own homes.

The successful candidate will be expected to support the Registered Manager in ensuring the safe, effective delivery of care services. The qualified candidate must hold a L3 / L4 Qualification in Health and Social Care or be ready to work towards L4 or L5.

Support will be required in the following areas:

-Assist with day-to-day operations, including support to other branches as needed.

-Support the care manager in interviewing and selecting new professional carers, and coordinate with multidisciplinary teams to ensure seamless care delivery.

-Assist with scheduling care visits.

-Attend client reviews and update care plans and risk assessments monthly or as required.

-Participate in new client meetings, complete initial assessments, and develop care plans and risk assessments.

-Maintain all client records in the care App and create corresponding office files.

-Coaching and supervising staff and promoting a high level of satisfaction.

-As a Care Coordinator, you will work closely with health professionals, agencies and the Local Authorities to secure care packages, provide effective and holistic care, making sure that their care is Safe, Effective, Caring, Responsive and Well-Led. Bonuses will be offered based on your input in growing the company. You must be proactive and have the Knowledge to win packages and source private care packages.

-You will also have the opportunity to receive further professional and management training.

-You will be able to cover last-minute cancellations and ad hoc shifts to ensure the safety of our clients.

A minimum of 1 year of relevant experience in domiciliary care services is required. The candidate must hold appropriate qualifications and have the capabilities to expand the business, using evidence-based practice skills and knowledge.

You must have a driving licence and own a car

Must be ready to work for two weekends in a month, either with clients or on-call
