Summary

Coordinate care for patients coming through the 2WW Pathway. Work with a range of clinical and non-clinical staff in the cancer pathway. Provide and receive complex information about patient care and interpret the patient plan for other clinicaland non-clinical staff. A key aspect of this role involves daily communication with patients, relatives and carers and other healthprofessionals. The information and nature of communication required is sensitive due to the nature ofcancer and therefore communication in this context requires a high degree of empathy, understanding,diplomacy, honesty, and integrity. To have a good knowledge and understanding of the Thoracic Cancer Pathway and an understanding ofthe radiology tests arranged, and further tests that may subsequently be requested. To provide non-clinical advice and information using good communication skills to navigate patientsthrough what will be a time of great anxiety and where patients may be highly distressed. Assist with requesting extra investigations as required e.g., blood tests. Following up and tracking results and acting on results in a timely, appropriate manner. Support the delivery of patient education using Making every Contact Count. Provide basic advice and refer on or signpost to other sources of support. Co-ordinate and organise appointments and assessments as required and to exchange that informationclearly and concisely dealing with anxiety, cultural or language difficulties. Plan and organise complex programme for co-ordination of patient pathway. Co-ordinate handover with other Teams to facilitate safe and effective transition of care between servicesto provide seamless support for patients. Facilitate communication between different clinical services to provide speedy service for patients withsuspected cancer. Document and monitor all aspects of care coordination and service delivery, supporting data collection foraudit. To ensure that high standards of documentation and care planning for patients are maintained.
