Summary

Senior Practitioner - Discharge to Assess

Location: South Ayrshire

Salary: £25,541 to £27,200 per annum

Hours: 35 per week

Contract: Permanent

Additional Requirement: Full UK driving licence required and access to a vehicle - business milage paid

Working closely with the Service Manager, you’ll ensure service users receive compassionate, person-centred support while guiding and mentoring staff to grow in their roles. What will a day in the life of a Senior Practitioner involve?

Coordinating referrals and liaising with hospitals, GP surgeries, and community teams.

Supporting and mentoring staff, students, and volunteers to deliver excellent care.

Conducting risk assessments and welfare checks to ensure safe living environments.

Managing rotas, responding to out-of-hours queries, and maintaining service continuity.
