Summary
The post holder will: Lead on horizon scanning activities and identify local, national and international best practice to inform transfer of care/discharge services and pathways across Organisations and where appropriate ensure that it is adopted within the portfolio Extensive experience in synthesis and analysis of highly complex data from national and local sources and translating data to the Board Usecomplex data and skilled judgement in interactions with stakeholders to shape the design of the portfolio Promote innovation and research across transfer of care/discharge services, ensuring any research is translated into best practice
