Role: Referrals and Partnerships Manager
Salary: £42,000 – £45,000 per annum + bonus
Location: North, National Travel
Your Role
Acting as a key point of contact for enquiries and referrals, providing a clear, consistent and professional experience throughout the referral journey.
Building and maintaining trusted relationships with commissioners, brokerage teams, care professionals and other referral partners.
Coordinating referrals and assessments, maintaining oversight of progress and supporting managers and assessors to achieve agreed timescales.
Working collaboratively with operational colleagues to understand individual needs, service suitability and any adjustments required to support a successful placement.
Supporting discussions with funding authorities and helping to ensure appropriate service proposals, agreements, contracts and transition arrangements are in place.
Maintaining a strong understanding of our existing services, vacancies, upcoming developments and potential discharge risks.
Promoting available and developing services through professional networks, conferences, exhibitions and targeted local activity.
Monitoring referral, enquiry and occupancy information to identify emerging demand, geographical gaps and opportunities for development.
Providing informed advice to operational and senior leaders on referral priorities, service fit and activity required to support occupancy.
Working flexibly with colleagues across the wider Referrals and Partnerships Team, including supporting referrals and assessments in other regional locations when required.

About You
To be successful in this role, you will:
Experience working at a management level within a social care environment.
Experience supporting people with learning disabilities.
A relevant Level 5 qualification, or equivalent experience and a strong professional track record.
Excellent written and verbal communication skills, with the ability to engage and influence a wide range of stakeholders.
Strong organisational and time-management skills, with the ability to manage multiple referrals, assessments and partnership activities.
Experience analysing information, identifying trends and producing meaningful reports and insights.
A sound understanding of person-centred practice, safeguarding, confidentiality, equality and diversity, and current social care requirements.
The ability to work independently, use initiative and contribute positively as part of a wider team.
A flexible approach to workload and travel, supported by a full UK driving licence.
