The post holder will undertake routine GP consultations and participate in the practice duty/on-call system as part of the agreed job plan. The post holder will work collaboratively with the wider multidisciplinary team and contribute to clinical governance, quality improvement, teaching, training and the ongoing development of the practice. As Oakfield Street Surgery is a GP training practice, the post holder will be encouraged to contribute to the teaching, supervision and development of learners within the practice, appropriate to their experience and role. Main Clinical Responsibilities The post holder will: Provide high-quality, evidence-based general medical care to registered patients. Undertake face-to-face and telephone consultations as appropriate. Assess, diagnose and manage a broad range of acute and chronic medical conditions. Provide appropriate follow-up and ongoing management of patients. Provide preventative care and health promotion. Prescribe safely and appropriately in accordance with current guidance and practice procedures. Make appropriate referrals to secondary care and other health and social care services. Maintain accurate, contemporaneous and appropriate clinical records. Communicate effectively with patients, relatives and carers. Ensure patients are treated with dignity, respect and compassion. Recognise and respond appropriately to safeguarding concerns. Work within the limits of professional competence and seek advice where appropriate. Duty / On-Call GP Responsibilities Participation in the practice duty/on-call system will form part of the agreed job plan. Responsibilities will include: Telephone and clinical triage of patients. Assessment and management of urgent clinical presentations. Providing appropriate urgent clinical advice and treatment. Undertaking appropriate home visits. Reviewing and responding to out-of-hours reports. Reviewing pathology, investigation and other clinical results. Reviewing relevant clinical correspondence. Providing clinical advice and support to the wider practice team. Arranging appropriate follow-up, referral or onward care where required. Responding appropriately to clinical emergencies. Communication and Team Working The post holder will: Work collaboratively with GP Partners, Salaried GPs and the wider multidisciplinary team. Maintain effective communication with practice nursing, pharmacy, administrative and reception staff. Provide appropriate clinical advice and support to colleagues. Communicate effectively with external healthcare professionals and services. Participate in relevant practice meetings. Contribute to a positive, supportive and respectful working environment. Communicate clearly and sensitively with patients and their families/carers. Teaching, Training and Professional Development Oakfield Street Surgery is an established GP training practice with a strong commitment to education and professional development. The post holder will be encouraged to: Support the teaching and training ethos of the practice. Contribute to the education and supervision of GP trainees, medical students and other learners where appropriate. Share knowledge and clinical experience with colleagues. Participate in continuing professional development. Maintain professional competence and skills relevant to the role. Participate in appraisal and revalidation processes. Identify personal learning and development needs and undertake appropriate activities to address these Clinical Governance and Quality Improvement The post holder will contribute to the practice's clinical governance arrangements and continuous improvement of services. This will include: Participating in clinical audit and quality improvement activities. Contributing to significant event reviews and learning from incidents. Supporting patient safety initiatives. Following agreed clinical guidelines, protocols and practice procedures. Contributing to the development and review of clinical policies where appropriate. Reporting clinical incidents, concerns and risks in accordance with practice procedures. Maintaining awareness of current clinical guidance and relevant NHS Wales requirements. Contributing to learning and service improvement arising from complaints, incidents and patient feedback. Safeguarding The post holder will: Recognise and respond appropriately to safeguarding concerns involving children and adults. Follow practice safeguarding policies and procedures. Maintain appropriate safeguarding knowledge and training. Make appropriate referrals and communicate concerns to relevant services. Work collaboratively with other agencies where required to support patient safety and wellbeing. Confidentiality and Information Governance The post holder will: Maintain patient confidentiality at all times. Comply with relevant data protection and information governance requirements. Ensure clinical information is recorded, stored and shared appropriately. Access patient information only where there is a legitimate clinical or professional reason to do so. Comply with practice policies relating to the use of clinical systems and patient information. Professional Responsibilities The post holder will: Maintain full GMC registration and a licence to practise. Maintain inclusion on the All-Wales Medical Performers List. Practise in accordance with GMC professional standards and guidance. Maintain appropriate professional indemnity arrangements. Maintain clinical competence and undertake appropriate continuing professional development. Participate in appraisal and revalidation. Work within professional and personal competencies. Comply with relevant legislation, professional guidance and practice policies. Promote equality, diversity and inclusion in all aspects of professional practice. Maintain appropriate standards of professional conduct. Health, Safety and Risk Management The post holder will: Comply with practice health and safety policies and procedures. General This job description is intended to provide an outline of the main duties and responsibilities of the post. The post holder may be required to undertake other reasonable duties appropriate to the role and level of responsibility. Terms and Conditions Working pattern: 46 sessions per week, MondayWednesday. The exact working pattern will be agreed with the successful candidate. Contract: Initial 12-month fixed-term contract, with a view to permanency. Start date: As soon as reasonably practicable, subject to contractual notice requirements. Salary: Competitive and commensurate with experience, reflecting current Welsh Salaried GP arrangements. Pension: NHS Pension Scheme. An individual job plan will be agreed with the successful candidate and will reflect the agreed sessional commitment and responsibilities.
