Summary

The following are the core responsibilities of the Non-Prescribing Practice Nurse. The postholder will work across both practice and community settings, delivering high-quality, patient-centred nursing care. Additional duties may be required in response to service needs, workload demands, and staffing requirements. Promote health improvement, disease prevention, and wellbeing through evidence-based health education, health promotion, and lifestyle support. Assess, plan, implement, and evaluate person-centred nursing care for patients with acute and long-term health conditions within own scope of practice. Support the management of patients with chronic diseases, undertaking routine reviews, monitoring, and follow-up in accordance with local and national guidelines. Identify patients at risk of developing long-term conditions and provide proactive interventions to improve health outcomes. Deliver routine nursing procedures including wound care, cervical screening (where competent), vaccinations, immunisations, health checks, venepuncture, ECGs, and other clinical interventions within competency. Undertake autonomous home visits, care home visits, and community-based assessments as a core component of the role, managing a delegated caseload and providing assessment, treatment, monitoring, and care planning for housebound and vulnerable patients. Manage an independent clinical caseload across practice and community settings, prioritising workload appropriately and exercising professional judgement when assessing, planning, and delivering patient care within professional competence and organisational policies. Develop and maintain therapeutic relationships with patients, carers, families, and multidisciplinary teams to facilitate coordinated care. Support the identification and reduction of health inequalities, with particular focus on improving access, engagement, and outcomes for patients with learning disabilities. Lead or support annual health checks for patients with learning disabilities, ensuring reasonable adjustments are made to meet individual needs. Advocate for patients with learning disabilities and work collaboratively with carers, community teams, social care, and specialist learning disability services to promote equitable healthcare access. Contribute to the development of personalised care plans, including support plans for patients with complex health and social needs. Provide health promotion, education, and self-management support to patients, carers, and families in ways that are accessible and tailored to individual communication needs. Request and undertake agreed diagnostic tests and investigations within scope of practice, reporting findings to the appropriate clinician and escalating concerns as required. Recognise, assess, and appropriately escalate clinical concerns, safeguarding issues, deteriorating patients, or signs of abuse, neglect, or vulnerability. Refer patients to other healthcare professionals, community services, social prescribers, and support organisations in line with practice protocols and local pathways. Maintain accurate, contemporaneous, and legally compliant clinical records using the practice clinical system. Ensure appropriate use of clinical coding and contribute to the maintenance of disease registers, enhanced services, and quality improvement initiatives. Participate in infection prevention and control activities, clinical audits, service development projects, and quality assurance programmes. Chaperone patients where appropriate and in accordance with practice policy. Support the practice team in responding to medical emergencies and urgent clinical situations. Recognise and respond appropriately to patients presenting with mental health needs, referring to relevant services where required. Actively participate in multidisciplinary team meetings, safeguarding reviews, case discussions, and care planning processes. Maintain professional registration and competency through continuing professional development, mandatory training, clinical supervision, and reflective practice. Contribute to the education, development, and support of colleagues, healthcare assistants, students, and trainees where appropriate. Work flexibly across practice and community settings to meet the needs of the patient population and support service delivery priorities. Special Interest: Learning Disabilities The postholder will be encouraged and supported to develop and maintain expertise in the care of patients with learning disabilities, including: Supporting delivery of the Learning Disability Enhanced Service. Undertaking and coordinating annual health checks. Ensuring reasonable adjustments are identified, recorded, and implemented. Promoting accessible communication and health information. Working collaboratively with community learning disability teams and external agencies. Supporting health action plans and personalised care planning. Helping to reduce health inequalities experienced by people with learning disabilities. Acting as a practice resource and advocate for learning disability awareness, inclusion, and best practice. Supporting carers and families to access appropriate healthcare services and community support. Promoting the uptake of screening programmes, vaccinations, health reviews, and preventative healthcare interventions among people with learning disabilities
