Summary

CLINICAL Perform comprehensive pain assessment of patients, plan, implement and evaluate care delivery according to changing health care needs and varying levels of complexity with patients who have pain management difficulties. Where the post holder is an independent prescriber, will prescribe medications within their scope of practice and competency for patients in their care, following assessment of need and in line with locally agreed guidelines and formularies Collect, collate, evaluate and report information, maintaining accurate patient records. Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to pain management plans and on-going care needs. Work autonomously on a regular basis and manage own case load of patients. Participate in clinical nursing duties and deputise for the Clinical Nurse Specialist in her absence. Recognise and respond appropriately to urgent and emergency situations. Deputise for the Clinical Nurse Specialist in the management and organisation of the department. Support the Clinical Nurse Specialist to maximise best use of clinical resources, in implementing best practice and taking responsibility for initiating improvements. Ensure the effective and efficient use of physical and financial resources. Participate in patient programmes including self- management and pain management programmes Participate and assist in out -patient treatments, to include providing TENS machines, External Neuromodulation and invasive pain treatments. Provide support and advise to patients regarding these treatments COMMUNICATION Involve patients and carers/relatives in the planning and delivery of pain management plans and act as a resource and a point of contact for this group of patients. Establish and maintain effective communication with patients and carers/relatives. Maintain accurate records of pain management plans and treatments/care given. Ensure appropriate information is given to the patients and/or relatives regarding the continuing plan of care and treatments ordered and assess the impact of such plans. Responsible for promoting and development of open communication networks and collaboration with colleagues, other professionals and agencies working across several disciplines. TRAINING AND DEVELOPMENT Responsible for pursuing own professional development and undertaking appropriate clinical practice skills as appropriate whilst having an awareness of the requirements. Participate in induction programmes for new members of staff within the chronic pain unit. Expected to provide mentorship and perceptorship for newly appointed staff and junior staff. To establish and maintain a regional and national network of contacts. To keep up to date with annual mandatory training. To discuss annually present performance and future developments with the CNS through a formal appraisal. Act as a resource for other hospital and community staff providing advice, nursing expertise and specialist knowledge as required. Attend study days relevant to pain management. RESEARCH & AUDIT To ensure practice is evidence based To promote and disseminate research and audit findings To carry out audit and evaluation in order to monitor the effectiveness of current practices and to improve health outcomes To participate in on going national and local research and audit projects Publish and present own research findings or developments in care locally, nationally and internationally as appropriate To demonstrate continual evaluation of practice and contribution to patient care/outcomes within the defined specialist nursing area, making agreed changes where appropriate
