An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for a skilled person who can do 21 hours a week (1 overnight shift- 11 hours and 2 twilight shifts- 10 hours) to work across the Hounslow locality based at Clayponds Hospital on a Fixed Term Contract for 6 months to cover maternity. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight).
Please note that the service is currently undergoing service re-design and the operational hours will change by 7thDecember 2026 from 7pm-7am to 8pm-8am under phase 1 transformation project. The phase 2 (TBC) will be happening shortly after the implantation of phase 1 with the Night Service re-design involving the twilight and the overnight shifts. We are looking for someone who will be very flexible in supporting the new service structure in the future.
The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.
To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.
To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient’s condition in a timely way to the registered nurse.
Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.
To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.
