Summary
Main Duties and Responsibilities The successful candidate will be expected to independently manage and review patients across a range of long-term condition disease registers, including: Diabetes Asthma COPD Hypertension Coronary Heart Disease Chronic Kidney Disease Stroke/TIA Heart Failure Learning Disabilities reviews Other long-term conditions as appropriate Additional responsibilities will include: Qualified and competent in the fitting, removal, and management of Long-Acting Reversible Contraception (LARC), including contraceptive implants (Nexplanon/Implanon) and intrauterine contraception (coils). Evidence of relevant accredited training and maintenance of competency in LARC procedures. Conducting annual reviews and monitoring of patients with chronic diseases. Developing and implementing personalised care plans. Undertaking clinical assessments and examinations. Prescribing medications within competence and scope of practice. Reviewing and adjusting treatment plans in line with national and local guidelines. Delivering health promotion and preventative healthcare interventions. Vaccinations and immunisations. Cervical cytology. Supporting Quality and Outcomes Framework (QOF) achievement as well as local and national incentives. Maintaining accurate clinical records and coding. Participating in practice clinical audits and service improvement initiatives. Essential Criteria Registered Nurse (NMC registered). Independent Nurse Prescriber qualification. Experience working within General Practice or Primary Care. Proven experience managing long-term condition clinics and disease registers. Strong clinical assessment and decision-making skills. Excellent communication and interpersonal skills. Ability to work autonomously and as part of a multidisciplinary team. Proficient in clinical IT systems and electronic patient records. Desirable Criteria Chronic Disease Management qualifications. Cervical Cytology qualification. Spirometry qualification. Diabetes diploma or equivalent. Experience of EMIS Web.
