Summary

Thank you for your interest in this post for as aCare Home Practice Foundation Pharmacistin the Care Home GP Practice.

The Care Home GP Practice is part of Ealing Community Partners (ECP) and provides an enhanced primary care service to Care Homes (Nursing and Residential) across Ealing. The team includes GPs, Pharmacists, Nurses, Advanced Practitioners and Administrators and the service is provided 8am – 8pm 7 days per week.

The service works closely with colleagues across ECP and Ealing’s Borough Based Partnership teams as well as Care Home providers with the aim of providing an integrated approach to the care available to the residents.

The service has transferred to the Trust, from a previous subcontracted arrangement, and this will give the opportunity to explore the current and future delivery of the service via a transformation programme.

The ECP Care Home GP Practice providesenhancedprimarycareservices for over 1000 patients in nursing homes and care homes in the borough of Ealing. 

The post holder is a pharmacist, who acts within their professional boundaries, working as part of a multi-disciplinary team in a patient-facing role. The post holder will assist in areas of patient care and chronic disease management within the care home practice, undertaking clinical medication reviews to manage patients especially for polypharmacy, older people, residential and nursing home residents with multiple co-morbidities.

The post holder will provide support to pharmacy team and care home general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing advice on clinical medicines issues while addressing both public and social care needs of patients in primary care.

The post holder will provide clinical advice on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
