Summary

The Specialist Community Forensic LD Team or CLDP currently covers the five London boroughs of Camden, Islington, Barnet, Enfield, and Haringey, and aims to provide both assertive pre-discharge (transition management) and high levels of multi-disciplinary community forensic after-care for restricted or unrestricted learning-disabled patients discharged from secure care. Additionally, the post holder will be expected to undertake assessments of LD patients referred to the North London Forensic Service for risk assessment or advice, and will attend professionals multiciliary meetings with generic community teams that refer services users to them. The referrals come from mainly from generic community teams. The post holder will also be expected to join the NLFS consultant group in assessing new external referrals to the service, averaging 9 per year. This includes face-to-face assessments and also joining the Forensic Risk Advice Panel. The post holder will be expected to support the Trusts Clinical Governance Framework for monitoring and improving standards of care through taking part in activities for improving quality and safety, assisting with NHS PSIRF process, identifying and managing risks within the service, as well as maintaining his or her own continuing professional development.
