Summary

Participate in regular MDT meetings and clinical supervision as required Promote evidence-based practice using the latest research-based guidelines and participate in practice-based research where appropriate. Provide a holistic service to patients, their families and carers, developing, where appropriate, an on-going plan of care/support, which includes advice on prevention and self-care. Promote health and well-being using health promotion, health education and therapeutic communication skills. Record contemporaneous, accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards. Taking responsibility for own development, learning and performance Make professionally autonomous decisions for which he/she is accountable.
