Summary

Home First Hub is an integrated discharge team working with our Acute hospitals across Norfolk to support patients through their discharge home, community inpatient unit or to residential placement.

It is a multidisciplinary team of clinical and social professionals dedicated to patient recovery and wellbeing. EEC is seeking an experienced and enthusiastic Occupational Therapist to join our team in providing reablement assessment to a specific cohort of patients across a 5 day service Monday to Friday.

This role will focus on patients who are not able to immediately return home or enter a community in patient unit but require a period of recovery, reablement, and assessment to understand their long term needs and goals. It will involve linking with our team at Norfolk & Norwich University Hospital and visiting patients in their short term placements in residential homes across Norfolk.The specific cohort of patients will be those predominantly with cognitive impairment

· To contribute to the delivery of high standards of health care to patients within care homes by assessing, planning, implementing and evaluating packages of care.

· Beresponsiblefordeliveryof careinareablementand recoverysetting.

· To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.

· Tosupportpatientsbeingasindependentaspossiblewithintheirfunctionallimits.

· Toworkaspartofamultidisciplinaryteamwithinaqualityframeworktoensurethatthehighest standards of care are met.

· Tosupportanddeliverinnovationwithindischargepracticeandpathways.

· Toworkwithsystempartnersincludingthevoluntarysectortoensurethatpatientsexperience isaseamlesstransferofcareback home or to a long term placement.

· Beresponsibleforthedeliveryandco-ordinationofcaretoanamedgroupofpatients.

· Workproactivelywithintheteamtoachieveunitandtrustgoalsandobjectives.

· Workwithandpromoterecognisedpolicies,protocols,andguidelines.

· Work with HFH team to identify opportunities for learning and improvement specific to this patient cohort.
