Summary
Job Purpose To provide high-quality, patient-centred nursing care within a busy GP practice, supporting the prevention, diagnosis and management of health conditions while promoting health and wellbeing. The post holder will work collaboratively within the multidisciplinary team to deliver safe, effective and compassionate care. Main Duties and Responsibilities Deliver a range of nursing services within scope of practice, including wound care, cervical screening, immunisations and vaccinations. Undertake health promotion activities and provide lifestyle advice to patients. Support the management and review of patients with long-term conditions such as diabetes, asthma, COPD and hypertension. Perform clinical assessments and provide appropriate care, treatment and referrals. Maintain accurate and contemporaneous clinical records. Participate in infection prevention and control activities. Contribute to quality improvement, clinical governance and service development initiatives. Work collaboratively with GPs, healthcare assistants, pharmacists and other members of the multidisciplinary team. Support teaching, training and research activities where appropriate. Maintain professional registration, mandatory training and continuing professional development.
