Summary

The Nursing Team supports a diverse patient population of approximately 10,000 registered patients and works collaboratively with GPs, Practice Nurses, Healthcare Assistants, administrative staff and other members of the multidisciplinary team. Services provided by the team include: Chronic disease management NHS Health Checks Hypertension management Contraception and sexual health services Cervical cytology Wound care and ulcer management Childhood and adult immunisations Phlebotomy ECGs Health screening and health promotion Well-Women clinics Minor Surgery clinics The team works collaboratively within a multidisciplinary environment to deliver safe, effective and evidence-based care. There is a strong emphasis on teamwork, clinical supervision, training, competency development and continuous improvement. Key Roles and Responsibilities Clinical Care Undertake delegated clinical tasks safely and effectively within the scope of the HCA role and individual competency. Perform general health checks and health screening in accordance with practice protocols. Accurately measure, record and report patients' height, weight, blood pressure, pulse, temperature and other agreed observations. Calculate and record Body Mass Index (BMI) where appropriate. Perform ECGs following appropriate training and competency assessment. Undertake venepuncture/phlebotomy following appropriate training and competency assessment. Provide wound care and dressing changes in accordance with agreed treatment plans and practice protocols. Remove simple dressings and undertake other delegated wound-care activities within competence. Recognise and report abnormalities, deterioration or concerns to the appropriate registered healthcare professional. Support clinicians with examinations, procedures and treatments where appropriate. Assist with specimen collection and preparation in accordance with practice procedures. Maintain accurate and contemporaneous clinical records following all patient contacts. Ensure that patients are treated with dignity, respect and compassion at all times.
