Summary
Main duties of the job Support the management of long-term conditions, including diabetes, asthma, COPD and ischaemic heart disease working closely with the nursing team Provide appropriate health promotion for patients with or at risk of a long- term conditions Manage & treat long-term conditions in line with the most current national & local guidelines Cervical smear taking Providing all immunisations, including: - Routine childhood vaccinations - Adult vaccinations - Seasonal vaccinations (e.g. flu, COVID-19) - Travel vaccinations and travel health advice Supporting the practice in achieving QOF targets and enhanced services Contribute to the provision of education & training for other staff & students Participate in research & audit where appropriate Maintaining accurate and contemporaneous records at all times Attend and participate in any meetings when required Any other delegated duties appropriate to the role Flexibility of working to ensure sufficient cover is provided for nurse holidays/sickness Maintain effective liaison with other agencies & staff concerned with patient care
