Summary

We are seeking to appoint dynamic and enthusiastic candidates to the role of Patient Pathway Coordinator, located at Cedar House, within our St. Michael's Community Hospital site within the Child Development Service Administration team. 

There are 2x roles offered, with one being:

1. 3x days per week: Monday, Thursday, & Friday (22.50 hours)

2. 2x days per week: Thursday & Friday (15.00 hours)

The appointed candidates will provide a high-level administrative service to the team of nursing staff to ensure the best level of care can be provided to all patients and service users who come into contact with the team. Training will be provided as required, and this role is a perfect opportunity for candidates wishing to pursue a career in the NHS within Operations.

The postholder will join a high performing administration team maintaining all patient pathways within the Enfield Community Services – Child Development Service Administration team, working alongside our Consultant Community Paediatricians, Community Nursing team, Community Allied Health Professionals, and support staff from across the Health Unit.

The postholder is responsible for ensuring the safe and efficient management of the patient throughout their pathway; giving the patient a single point of contact for their appointments, ensuring the patients and carer experience is central to every stage of the patient journey, and ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager. 

The postholder will proactively manage and track the progress of all referrals to ensure each stage of the pathway is fully planned and booked, whilst also working with colleagues throughout the Trust to ensure waiting time targets are achieved. They will be required to assist in the data quality and validation, ensuring all patients have recorded pathway start dates, and support the management team to identify areas that require pathway improvements.

North Mid is part of North Central London integrated care system – consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey. As with other ICS’s, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.
