• To support a 17-year-old young man who has Cerebral Palsy, learning disability, ADHD, Autism and Epilepsy to access the community and to give the main carer a break.

Main Duties:

• Access the community

• Support to visit the Cinema

• Support to go swimming

• Out for the occasional meal
