Summary
To provide high-quality, person-centred care and support to individuals living in supported accommodation and in their own homes, promoting independence, dignity, wellbeing, and positive outcomes.
Key Responsibilities
Deliver personal care and practical support in line with individual care and support plans.
Support individuals with daily living skills, including personal care, meal preparation, budgeting, shopping, and household tasks.
Encourage independence and support access to education, employment, healthcare, and community services.
Maintain accurate records and report any concerns or changes in an individual's wellbeing.
Follow safeguarding, health and safety, infection control, and medication procedures.
Build positive relationships with individuals, families, colleagues, and partner agencies.
