Summary

Surrey Downs Health & Care

We are looking for a dedicatedRehabilitation Assistantto join our community therapy team. In this role, you will support the delivery of rehabilitation services across physiotherapy, occupational therapy, and speech and language therapy. Working in a variety of settings—including patients’ homes, clinics, and community spaces—you will play a key part in helping individuals regain independence and improve their quality of life.

 

• To provide rehabilitation support services.

• To undertake specific skilled support work to aid the rehabilitation of patients in the Community.  Treatment may occur in the department, patients’ home or other appropriate setting. Treatment may be provided for Physiotherapy, Occupational Therapy or Speech and Language Therapy. 

• To carry own patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist. Monitor progress and make minor alterations to patient treatment programmes as required.

• Work unsupervised reporting back patient progress and informing the treating therapist of any problems. Receiving guidance and advice on an almost daily basis

To gather information prior to seeing client and provide feedback to the therapist.

To contribute to the assessment process and provide feedback to the therapist.

To implement therapy programmes/approaches, both on an individual basis and for groups and provide feedback to therapist.

To contribute to the evaluation of therapeutic interventions, including discharge planning.

 To be able to prioritise workload, following managerial supervision.
