Summary
General Requirements Maintain NMC registration and adhere to professional guidelines and code of conduct Work within the framework of relevant policy and legislation both local and national Principal Duties Clinical Triage and determine urgency of new referrals to the service Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action To assess, evaluate and manage clinical risk To formulate a plan of care based on identified need Deliver evidence based interventions as appropriate Hold and manage own caseload consisting of people with more complex needs To identify, monitor and review physical health and contribute to the management of this Review effectiveness of care plan at regular intervals If a non-medical prescribing qualification is held undertake prescribing as appropriate to the needs of the service and in line with the services protocols To work collaboratively with service users, carers and partner agencies To maintain computer held care records in an accurate and timely fashion Undertake all work in a culturally sensitive manner Act in accordance with the Care Programme Approach, Acute and Schedule Care Pathways as appropriate. Develop effective working relationships with partner agencies Supervision Regularly review own caseload with supervisor to promote efficacy and contribute to the overall management of the team Deliver supervision and facilitate annual PDR with more junior members of staff Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution Education and Training Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status where applicable Participate in training relevant to role Promote insight and understanding of mental health issues to service users, carers and other agencies Contribute and lead in audit, research and evaluation as appropriate Leadership and Service Development Take a lead in and contribute to service development and improvement Deputise for the Team Manager in their absence Effectively co-ordinate and manage the day to day workload of the team
