Summary

To work as a senior dietitian, providing evidence-based nutritional information and support to patients and their families across primary and secondary care. Manage a delegated caseload of paediatric patients with a variety of acute and chronic diseases with clinical support from specialist paediatric dietitian. Ensure their treatment is evidence based and patient centred. Assess, plan and implement interventions. Clinical caseload: Acute paediatric ward, Weight management, faltering growth, non-IgE mediated allergy. Act as an independent practitioner and be professionally and legally accountable and responsible for all aspects of their own work. Provide Dietetic treatment to a delegated caseload using evidence-based patient centred principles to assess, implement and evaluate interventions. Interpret medical, biochemical and pharmacological information, and incorporate into dietetic plans as appropriate. Recommend and discuss treatment options with the Consultant Paediatricians/General Practitioners / Referring Agents/ specialist nurses. Develop nutritional care plans for a caseload of patients based on assessment of medical, social, cultural and psychological factors. Monitor and review the aims and targets. Act as a resource on aspects of general nutrition to patients, carers, other health and education professionals, including GPs, consultants, and student dietitians. To cover patients from the Dietetic team based with as caseload requires Liaise with supervising/Specialist Dietitians regarding complex patients to agree appropriate management. When appropriate liaise with members of the health care team and/or specialist Dietitians regarding further management of patients in your caseload once they have been discharged from your Dietetic care. Decide priorities for own work, balancing other patient related and professional demands in consultation with the Head of Nutrition & Dietetics. Recommend prescription of oral and enteral nutritional products to medical practitioners in primary and secondary care on behalf of patients and advise on their safe usage. Contribute to the delivery of education programmes to staff and patients within delegated caseload. One to one dietetic counselling, as well as group education to staff, patients and their families will be included in this post. To participate in dietetic/MDT/ specialist team meetings and case conferences to discuss the nutritional management of patients. Contribute to the training and development of Dietetic students and Dietetic Assistants. Support senior staff with service developments and input into policy, guideline and procedure development as well as departmental audits. This will include updating and development of nutrition and diet literature and resources and to ensure this is nonbiased and evidence based /best practice. To attend and participate in regular dietetic team meetings. To deliver dietetic care in accordance with Nutrition & Dietetic service standard, organisational policies, clinical governance, National Professional Standards of Practice and the Health & Care Professions Council (HCPC) Standards of Conduct, Performance and Ethics. Maintain appropriate, accurate and timely patient records in line with Nutrition & Dietetic service policy, GDPR, HCPC, BDA and Jersey Data Protection Law.
