Summary

Job Responsibilities Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Cytology Provide cervical screening and sexual health services as per contract requirements. Wound care / ulcer care/Doppler Removal of Sutures New Patient Medicals/Urinalysis ECGs Asthma/FENO Readings/Spirometry Venepuncture Hypertension Management Ear syringing Routine immunisations/Childhood immunisations Chaperoning and assisting patients where appropriate who are being examined by another clinician Requesting pathology tests, for example urine culture, swabs Following agreed clinical protocols with referral to senior nurses or GPs as appropriate Other tasks will include: Maintaining and cleaning equipment used by the nurses and GPs Maintaining GP and Nurses rooms, stocking and rotating items as required Maintain general tidiness and cleanliness of nurses and treatment rooms Maintain stock of waiting room leaflets as instructed by partners and Practice Manager Ordering of vaccinations to maintain stock levels Participation in administrative systems in the Practice At all times there will be a need to maintain accurate records A duty to advise senior nurses of potential problems or errors within a range of assigned tasks Attend and participate in any Practice meetings when required. Contribution To the implementations of Services: To support the Infection Prevention Control (IPC) Team/Lead to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the Practice Manager To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF Conduct chronic disease reviews (asthma, COPD) within your remit Ensure accurate coding and documentation to support QOF indicators Monitor and recall patients for annual reviews and follow-ups where required Support vaccination and immunisation programmes (e.g., flu, shingles) Support the team to improve QOF performance Support the Delivery of services under Enhanced Services (ES), Local Incentive Schemes (LIS), and Directed Enhanced Services (DES) within your remit Contribute to data collection and reporting for contract compliance As required, advise potential diagnosis and manage acute and chronic conditions within your remit To review patient medications to enhance compliance in accordance to own scope of practice To request pathology services and process pathology results as required To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required to maintain accurate clinical records in conjunction with current legislation To ensure read codes are used effectively To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately To assist GPs with minor surgery when required To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To assist vaccination programme clinics for adults and children The responsibilities described above are not exhaustive and may be amended from time to time in line with the needs of the practice. The post holder may be required to undertake other duties commensurate with the grade and scope of the role, subject to workload, staffing requirements and their level of competence.
