Job Title Mental Health Outreach Support Worker

Hours of Work Various part-time opportunities available; maximum 26 hours, minimum 12 hours – Monday to Friday

Responsible for The Mental Health Outreach Support Worker is as newly emerging role within the Age Well Teams, providing mental health support to patients within the community.

Location/Base  Opportunities available across the county, working from one of our Community Hubs

Job Summary:

The position of Mental Health Outreach Support Worker will be working in partnership with the Primary Care Network’s Age Well Teams. The role provides the opportunity to work innovatively with a range of partner organisations to enhance the experience of older adults living in care homes and their own home.

The Age Well Teams provide early multi agency support for emerging needs, including Frailty clinics and care home teams and are working in partnership with Northamptonshire Mind to include mental health support for their patients.

Experience of, or a real interest in, supporting older adults is essential.

Working as part of our overall Integrated Holistic Support Team your primary responsibilities will be to:

- Support patients identified through the team who need mental health or emotional wellbeing support.

- Support patients through structured interventions, providing tools and strategies to support individuals’ mental health and emotional wellbeing.

- Signpost patients to other 3rd sector services, support & other community resources that will benefit the patient and assist patients to access the services identified.

- Work with patients to build confidence to attend and engage in services and/or support identified.

- Review whether identified services and support are working for the patient.

- Maintain an individual case load whilst working cohesively alongside other Age Well partner organisations, supporting a collaborative approach

- Support patients to enable them to remain as independent as possible within their own home and reduce the risk of crisis support.

- Participate in any other work identified by the team that is deemed suitable for a Mental Health Outreach Support Worker

- Ensure all meetings/interventions with patients are recorded.

- Attend any meeting identified for the Mental Health Outreach Support Worker to attend, including Multi-Disciplinary Team meetings.

Experience and Skills:

Essential (E), Preferred (P)

- To have a positive attitude towards people with mental health issues (E)

- Demonstrate organisation and scheduling skills effective planning, to be creative and solutions focused. (P)

- Resilience and ability to deal with challenging situations. (E)

- Previous experience of working with vulnerable adults in a 1:1 setting and person-centred way. (P)

- Experience of creating and managing personal improvement/wellbeing plans for service users. (P)

- Ability to work within professional boundaries. (E)

- Computer literate (E)

- Full Driving Licence (E)

What we offer you:

- 31 days leave per year including public holidays, rising to 33 days after 5 years’ service, pro rata for part-time employees.
