Summary

To be responsible for the unsupervised assessment of patient care needs in the Lung Heath Check service, ensuring appropriate assessment, planning, implementation and evaluation of care, maintaining high standards of evidence-based care. To provide expert clinical assessment for a defined group of patients, including those with complex needs ensuring they are placed through the correct MDT and clinical pathway.
