Summary

The post holder will: * Provide Physiotherapy assessments and interventions to individuals requiring Physiotherapy within the Complex Care at Home service. * Undertake all aspects of clinical duties as an autonomous practitioner with support and guidance from the Physiotherapy Professional Lead and Clinical Specialists Physiotherapists within the trust *Provide advice and support for all members of the MDT involved in the patient's care. For example, Matrons and Wellbeing Coordinators within the CC@H service. * Evaluate progress, re-assess and amend exercise and treatment programmes as required using specialist knowledge. To refer to other specialist services as required. The Trustis required towork in line with the Governments UK Visa and Immigration Rules and Regulations.
