Summary

The post holder will be a high-profile, dynamic nurse with a keen attention to detail and a holistic approach to care. As an accessible and responsive leader, they will hold 24-hour responsibility for patient care, staff coordination, service delivery, and resource management within a defined area. They will make informed decisions within their professional scope, involving patients and the public to meet agreed clinical standards and targets. The role includes managing and coordinating staff and clinical activities within the departments liaison team, playing a key role in the multidisciplinary team, and leading change management initiatives within the haemoglobinopathy service. Strong communication links with divisional operational management and other stakeholders will be essential to ensure staff are effectively briefed and supported. The post holder will provide specialist support and advice to healthcare professionals locally and regionally, working closely with patients to reduce hospital length of stay, promote independence, and encourage adherence to treatment plans and appointment attendance. They will lead a team of liaison sisters who support patients in the community, conducting home visits to assess individual needs and tailor support accordingly. Much of the work will focus on hospital avoidance, promoting compliance, and improving overall patient outcomes. A full UK driving licence and daily access to a personal vehicle are required, as the role involves occasional lone working during home visits, necessitating vigilance and thorough risk assessment. The post holder will be required to prescribe medications to patients therefore a having the non-medical prescriber qualification is essential.
