Summary
Your responsibilities will include: Delivering clinical care and patient education in patients homes and virtually Supporting patients, families and carers throughout their treatment Working with NHS hospitals and multidisciplinary teams to support safe discharge and ongoing care Leading, coaching and supporting a small clinical team Monitoring clinical quality, patient outcomes and service KPIs Maintaining accurate and timely clinical documentation Managing and escalating clinical incidents and adverse events appropriately Supporting training, development and performance within the team Participating in the on-call service as required
