Summary
Use the principles of Making Every Contact Count (MECC) to provide brief health and wellbeing interventions, to help people make informed choices and take positive steps to improve their health. Work with individuals to identify health and wellbeing needs and develop personal health plans and realistic action plans, reviewing progress and adapting goals to help people make and sustain positive behaviour changes. Plan and deliver engaging one-to-one and group sessions, providing advice, education and practical support around healthy lifestyles and wellbeing. Carry out basic health checks and observations, such as height, weight and blood pressure. Document health plans and maintain accurate patient records. Identify individuals who may benefit from additional support and signpost or refer them to appropriate health, wellbeing and community services. Build positive, supportive and non-judgemental relationships with individuals, carers and families, adapting communication to meet different needs and circumstances. Work collaboratively with colleagues, health professionals and external organisations to provide coordinated support and improve outcomes for individuals. Develop and maintain links with local community, voluntary and health organisations, including identifying new referral pathways and strengthening existing pathways into the Health Trainer Service. Collect data to contribute to the evaluation and improvement of service performance. Recognise and appropriately respond to risks or concerns relating to patient care, escalating issues to senior colleagues where required. Plan and manage your own workload effectively, ensuring agreed activities, appointments and targets are delivered within required timescales. Attend and contribute to relevant meetings, training, community activities and health promotion events. You must be able to travel across the city to meet service users and deliver sessions in a range of community settings. You must becompetent in phlebotomy, or be willing to develop this skill and achieve competency within three months of appointment. Your application is an opportunity to tell us about you.
