Summary
Essex Partnership University NHS Foundation Trust (EPUT) has an exciting opportunity to join the Intergrated Community Care Team based at Saffron Walden Community Hospital
This part time , role plays a vital part in supporting the delivery of safe, responsive, and high-quality care for patients across the community. The postholder will work as part of a dynamic, fast‑paced service that ensures individuals receive the right care, at the right time, in the right place.
The postholder will be responsible for:
• Acting as a central point of contact for calls and referrals into service
• Coordinating care and communicating effectively with clinicians, patients, carers, and partner agencies
• Maintaining accurate and timely records using Trust clinical systems
• Supporting multidisciplinary team working to ensure seamless patient journeys
• Escalating concerns appropriately and contributing to service flow and patient safety
• Providing excellent customer service in a fast‑paced and pressured environment
• Supporting service audits, data collection, and performance monitoring where required

• Excellent communication and interpersonal skills
• Ability to work effectively in a busy, time‑critical environment
• Strong organisational and prioritisation skills
• Confident use of IT systems and electronic patient records
• Ability to work independently and as part of a multidisciplinary team
• Understanding of confidentiality, data protection, and safeguarding principles
