Summary

Job Purpose To provide a wide range of nursing care as determined by service requirements and delegated by a registered practitioner at the level required of a Nurse Associate. All concerns and queries would be escalated to the Registered Nurse on duty who will provide advice and guidance. Duties and Responsibilities This list of duties is not intended to be exhaustive but indicates the main areas of work and may be subject to change after consultation with the post-holder to meet the changing needs of the organisation. Communication & Relationships Communicate effectively across a wide range of channels and with a wide range of individuals, the public, health and social care professionals, maintaining the focus of communication on delivering and improving health and care services. Demonstrate inter-personal skills that promote clarity, compassion, empathy, respect and trust and liaising with patients, staff and visitors at all times. Contribute to team success and challenge constructively to promote service improvements. Communicate with individuals, carers, family and other visitors in a courteous and helpful manner, whilst being mindful that there may be barriers to understanding. Report to appropriate registered care professionals information received from individuals, carers and team members to ensure the best possible care and to continue to promote multi-disciplinary working/partnership working. Support individuals, their families and carers when faced with unwelcome news and life changing diagnoses. Clinical Responsibilities Deliver high quality, compassionate care under the direction of a registered Nurse (or other registered professional dependent on the setting) with a focus on promoting health and independence. Be responsible and accountable for own practice, working within limits of competence and within professional boundaries while raising concerns to a registered care professional or appropriate person as and when required. Carry out specific delegated clinical tasks and responsibilities to a high standard and competency which promotes excellent patient care and continued partnership working. Where appropriate work without direct supervision, at times delivering care independently in line with the individuals plan of care, within designated parameters of practice of the Nursing associate role, accessing clinical and care advice when needed while providing feedback on progress against the care plan. Maintain understanding of all elements of the nursing process and be able to assist the registered nurse in on-going assessment, planning, management, and evaluation of care. Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence i.e. Making Every Contact Count. Manage and monitor the condition and health needs of individuals within their care following administration of medication, and to escalate as necessary. Undertake tasks that are relevant to the speciality including cannulation, venepuncture, including administration of medication as per trust policy. Perform and record clinical observation including blood pressures, temperature, respirations, pulse, peak and oximetry measurements and any additional requirements within speciality and competence. Accurately record nutritional and fluid intake to identify signs of dehydration or fluid retention and escalate as necessary. Use contemporary assessment tools e.g. Waterlow and nutritional assessments. Assist with feeding and drinking and support the delivery of artificial nutrition and hydration using oral and enteral routes as set out by Dietician and/or SLT Team. Measure and interpret blood glucose levels, recognise emergency situations, administer basic physical first aid, including BLS and observe and monitor comfort, pain levels, rest and sleep patterns. Use appropriate positioning and pressure relieving techniques, and pressure relieving / manual handling equipment. Monitor wounds and undertake wound care using appropriate evidence-based techniques, use aseptic non-touch techniques, care for, manage and insert catheters for all genders including all associated documentation. Review preferences and care priorities of the dying person and their family and carers, and ensure changes are communicated as appropriate, providing care for the deceased person and the bereaved respecting cultural requirements and protocols. Manage and monitor effectiveness of symptom relief medication, seeking the advice of a trained member of staff when required. Assist in the implementation of appropriate action to meet the specific physical, emotional, and psychological, social, cultural and spiritual needs of individuals, families and carers. Assist the Registered Nurse with complex care as deemed appropriate. Maintain an understanding of caring for individuals with particular conditions for example dementia, mental illness, learning disabilities. Ensure that privacy, dignity and safety of individuals is maintained at all times. Ensure that all precautions are taken to safeguard patients property and valuables. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals. Assist with the implementation and monitoring of clinical standards and outcomes. Demonstrate a good understanding of the Mental Capacity act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from Registered Nurse or other registered professional as required. Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures. Deliver effective care following plans determined by the registered nurse or other registered care professional and provide feedback on progress against the plan. Assessment and Judgement Skills Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other care professionals as appropriate. Report back and share information with the Registered Nurse on the condition, behaviour, activity and responses of individuals. Assist in the assessment of and management of risk across the environment where care is being administered. Exercise judgement in assessing patient condition, comfort and wellbeing using analysis of a range of possible factors. Information and Systems Ensure all patient related information is treated sensitively and adhere to the principles of confidentiality at all times, using Trust documentation or electronic records to record the outcome of assessment and evaluation of care. Report any accidents or incidents and raise concerns as per organisational policy using the appropriate systems such as Ulysses to ensure that all incidents are dealt with accordingly. Ensure clear, concise, accurate and legible records and all communication is maintained in relation to care delivered, adhering to local and national guidance. Ensure ability to manage electronic record systems taking cognizance of need to protect access (ID, passwords, swipe cards). Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met, within parameters of practice. Use Information Technology equipment including PCs, IPads & other such equipment on a regular basis. Service Development Support, supervise, teach (where appropriate) and act as a role model to trainee nursing associates, student nurses, healthcare support workers and new staff members, promoting reflection and proving constructive feedback. Share ideas with colleagues to improve care and suggest areas of innovation. Regularly attend workplace and staff engagement meetings and contribute positively to discussions about the improvement of care. Plan and manage competing demands of job role, continuous professional development and work-related activities. Personal Responsibility Exercise personal responsibility and work independently within defined parameters of practice, taking the initiative in a variety of situations and performing a range of clinical and care skills consistent with the role, responsibilities and professional values of a Nursing associate. Ensure that only those activities for which competence has been achieved are undertaken without supervision and that recognition is given to the impact and consequences of practising outside capability. Human Resources Demonstrate own duties to new or less experienced staff to ensure consistent ways of working and best patient care. Support development of less experienced staff and students to support their ongoing development requirements relevant to the role of ANP.
