Summary

Autonomous Decision-Making:Lead clinical assessments for undifferentiated and undiagnosed conditions. Preventive Care Leadership:Conduct disease risk screening and early intervention. Care Coordination:Develop personalised treatment plans and manage referrals. Patient Education:Provide counselling and health education that empowers patients. Drive practice excellence through: Quality Assurance:Maintain meticulous clinical records and participate in audit processes. Digital Integration:Utilize Systm1 for enhanced patient care and data management. Prescribing Excellence:Follow evidence-based formularies while ensuring patient safety. QoF & LCS Compliance:Contribute to Quality and Outcomes Framework and Locally Commissioned Services achievements. Influence the future of healthcare delivery: Multidisciplinary Teamwork:Collaborate with PCN, system colleagues and healthcare professionals. Clinical Supervision:Mentor junior doctors, F1/F2/ST trainees, and student placements. Practice Development:Contribute to policy development and clinical standard setting.
